About the Role As a Home Health Physical Therapist, you will work one-on-one with patients in their homes, helping them regain mobility, independence, and confidence. Requirements: Must be at least 21 years of age Graduate of an accredited Physical Therapy program (APTA/CAHEP) with a B.S., B.A., or M.S. degree Licensed or eligible for licensure as a Physical Therapist in the State of Colorado Valid, non-restricted driver’s license and reliable transportation Ability to work a weekend schedule as needed Minimum of two (2) years of experience required; home health experience preferred
