Public Health NurseEmployment Type: 0 - Full-time Regular Pay Range: $68,227.00 - 102,341.00Location: Castle Rock, COOvertime Exempt: N Elected Office / Department: PUBLIC HEALTH DEPARTMENTThe professionalPublic Health Nurse for the Family Support Program is a unique,relationship-based, public health nursing role which requires the nurse toestablish and maintain a trusting therapeutic relationship with each client,throughout all phases of the program. The nurse assesses the physical,emotional, social, and environmental needs of the family and provides healtheducation, referrals, and case management.This position fosters a relationship between nurse and client as a basisfor proactive change.This role workswith families referred by Department of Human Services.Generally, the hiring range is $68,227-$85,284 annually. Promotes the maintenance of health through teaching, conducting needs assessment and case management, direct nursing assessment, and resource coordination. This includes providing information and setting goals in prenatal and postpartum care, nutrition, mental health, parenting, child health and development, family planning, and life course development.Prioritizes multiple work assignments, including home visits with clients, thorough and accurate and timely completion of required nursing and programmatic documentation.Develops and maintains therapeutic relationships with a culturally diverse client population from a variety of socio-economic backgrounds, utilizing concepts of reflection and motivational interviewing; modeling client-centered, strengths-based, and solution-focused approaches.Maintains a program-specific active case load. Continues to monitor and support client engagement in the program and proactively implements strategies to retain clients in the program. Ensures and adheres to professional boundaries.Represents Douglas County Health Department (DCHD) as a health resource person in the community. Interprets public health programs and policies to individuals, families, community groups, schools, physicians, and other collateral community agencies and service providers.Assists clients in finding resources to meet needs and makes referrals for services as appropriate. Collaborates with related community agencies and service providers through client referrals, written and verbal communications, and case/team conferencesDemonstrates impeccable work ethic, high level of accountability, personal integrity, and transparency, provides high quality, individualized, client-centered care in accordance with the program model and guidelines, and the nursing process.Evaluates client progress toward stated goals. Modifies nursing intervention as appropriate.Consults and collaborates with other professionals involved in providing services to clients; provides timely and coordinated case management services with a variety of health, behavioral health, and social service providers.Completes assessments per required timelines and with a high degree of accuracy.Participates in case load review and analysis to identify achievements and areas for improvement. Participates in quality improvement efforts. Maintain confidentiality of information.Provide accurate testimony to the Courts, Child Protective Services, or law enforcement in reference to potential child custody, child abuse, or neglect situations.Performs other duties as assigned.SUPERVISIONRECEIVED:General supervision isprovided by the Assistant Director for Community Services, daily tasks andcaseload may be assigned by Human Services staff.SUPERVISORYRESPONSIBILITIES:NoneINDEPENDENTJUDGMENT:Exercises independent judgmentin formulating and implementing plans for nursing and general health care forindividuals and families based upon evaluation of needs and a focus on meetingmutually accepted health goals and objectives. Delivers nursing servicesthrough home visits and case management using excellent decision-making andproblem-solving abilities.MINIMUM QUALIFICATIONS:EDUCATIONand EXPERIENCE:Requires a bachelor's degree in nursing from a school accredited by the National League for Nursing Accrediting Commission or the American Association of Colleges of Nursing.One year of experience as a nurse in a similar environment required. A combination of experience and education may be considered.KNOWLEDGE,SKILLS AND ABILITIES:Knowledge:Thorough knowledge of the State laws, regulations, and programs relating to Family Support services. Thorough knowledge of the principles and practices of nursing services. Knowledge of prenatal and postpartum care, nutrition, mental health, parenting, child health and development, family planning, and life course development.Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.Skills:Is dedicated to meeting the expectations and requirements of internal and external customers. Gets first-hand customer information and uses it for improvements in products and services. Acts with customers in mind. Establishes and maintains effective relationships with customers and gains their trust and respect.Proficiency in performing physical assessments and using medical equipment.Must be able to learn and become proficient with equipment and software programs as required to perform assigned duties effectively and efficiently Excellent verbal and written communication skills. Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.Abilities:Relates well to individuals from diverse backgrounds and with unique needs. Ability to build rapport quickly. Builds constructive and effective relationships. Uses diplomacy and tact; can diffuse even high-tension situations comfortably.Ability to adjust actions in relation to others' actions.CERTIFICATIONS,LICENSES, & REGISTRATIONS: Requires an active license to practice as a Registered Nurse in the state of Colorado. Active certification in BLS and CPR are required.Must possess a valid Colorado Driver's License upon hire, with complying insurance. Review of motor vehicle record is required at time of hire and periodically throughout employment. This position requires successful completion of a criminal background check including fingerprinting through a national database.WORKENVIRONMENT:PhysicalWork Environment:Work is primarilyconducted in homes and community settings, with travel to other DCHD locationsor outside agencies to provide program services or attend meetings. Has close,in-person contact with co-workers and clients. While traveling to and workingin other locations, may experience exposure to noise, dust, solvents, fumes,smoke or gases, slippery/uneven walking surfaces, sun, road hazards, trafficconsiderations, motor vehicle accidents, and exposure to inclement weather suchas driving in snow, ice, etc. Potential for exposure to community relatedcrimes while on duty.PhysicalDemands:Position requires continual andextensive driving to client residences and other community-based settings;including high risk (increased crime) neighborhoods. Must be able to hear, talkand be heard/understood. Materialand Equipment Directly Used: Must demonstrate proficiency in using PPE;Computer use including internet, email, Microsoft Office Suite. Uses medicalequipment including, but not limited to, blood pressure cuff, weight scales,stethoscope, thermometer, syringes.Standard office equipment including, but not limited to, personalcomputer and associated hardware and software, telephone, copier, calculator,and fax.
