Bridges Community Health and Hospice is seeking a full-time Hospice RN Case Manager who is passionate about providing quality care to patients and families within the Castle Rock, Highland Ranch, Littleton area.Hospice RN Case Manager responsibilities include:Coordinating all aspects of the interdisciplinary care and services provided to hospice patients and their families.Provides education to families and caregiversDevelops, implements and reviews healthcare plansAdvocating for patients and their familiesDocumentation of careNursing assessmentsSymptom managementPromoting comfort and quality of life.Job Requirements:Current RN license to practice in the State of CO.Current CO driver's licenseReliable transportationCovid Vaccination CompleteMaintain current CPR certificationExcellent communication skillsMinimum of two years nursing experience with at least one year in hospice or home health preferred, but not required.Self-directed and able to work with minimal supervision.Demonstrate excellent observation, problem solving, verbal and written communication skills, nursing skills per competency checklist.Shows ability to organize and prioritize workload independently.Benefits Available:Medical/Dental/Vision401(k) matchingShort- and Long-Term DisabilityFlexible spending accountHealth savings accountLife insurancePaid time offReferral programScrub Allowance$70k - 85k annuallyBridges Community Health & Hospice has caring and dedicated staff that are passionate about helping people transition through the end-of-life process.
